Inflamed eruptions on the face, chest and back — described in Ayurveda as Yuvana-pidaka, "the pimples of youth", driven by aggravated Kapha-Pitta and meda (fat tissue) at the threshold of adolescence. Resolved with cooling face washes, Lodhradi lepa, and internal Manjishtha-Khadira.

“The pimples of youth” — and why the classical name matters

The Sanskrit name Sushruta gave this condition is Yuvana-pidaka — literally, “the affliction (pidaka) of youth (yuvana)”. The naming is precise: the condition is associated with a developmental window, not a permanent failing of the skin. That single observation contains the whole therapeutic philosophy — this will pass, if we don’t make it worse.

In Sushruta Samhita Nidana Sthana 13, the cause is described as the combined disturbance of Kapha, Vata, and Rakta (blood). Kapha contributes the oiliness; Vata the rough, painful nodules; Rakta the inflammation and redness. Modern dermatology gives us a more granular pathway — sebum overproduction (hormonally driven), follicular hyperkeratinization, Cutibacterium acnes overgrowth, inflammatory cascade — but the underlying recognition that acne is multifactorial and developmental is the same.

The classical protocol

Sushruta’s Chikitsa Sthana 20 prescribes a deceptively simple three-ingredient lepa for facial acne: Lodhra, Dhanyaka (coriander), and Vata-pallava (banyan leaves) — ground fresh with water, applied for 20 minutes, washed off. The synergy is: lodhra’s tannins astringe the follicle, dhanyaka cools, vata-pallava’s kashaya (astringent) rasa reduces oiliness.

For cystic, deep-seated acne, internal Manjishthadi kashayam is prescribed — a decoction of Manjishtha, Khadira, Nimba, and Haridra — taken twice daily for 30–45 days to clear raktadushti (blood vitiation). Modern science: Khadira’s catechins have well-documented antibacterial activity against C. acnes; Haridra’s curcumin is a known anti-inflammatory.

Diet is a non-negotiable part of the classical protocol. Abhishyandi foods — heavy, oily, sticky — are avoided. In modern terms: high-glycemic-index foods and dairy, both of which have meta-analysis-level evidence linking them to acne severity.

When this is more than youth

If you have nodular or cystic acne, if scarring is developing, if it has spread to your back and chest, or if you suspect a hormonal driver (PCOS pattern, perimenstrual flare on the jawline) — please consult a vaidya or dermatologist.
